How to Request a Quote
Enter your name, phone number and email, choose the type of coverage you are interested in, and add any details that help, such as your ZIP code, date of birth or current plan. Then click Submit. There is no obligation to buy anything.
- Your ZIP code, since plans vary by county
- Your date of birth or Medicare start date
- Your current coverage, if any
- Doctors you want to keep and medications you take
What Happens After You Submit
Kris reviews your request and reaches out by phone or email during business hours, Monday–Friday, 9:00 AM – 5:00 PM. She may ask a few follow-up questions, then prepare side-by-side comparisons. You can review them together in person at the Perrysburg office, or by phone or video.
For Medicare Advantage or Part D quotes, you will be asked to complete a Scope of Appointment, which you can find on the forms page. If you want to compare drug plans, the Rx drug lookup form is the quickest route.
